Important REGISTRATION Dates

  • Kindergarten Registration - March 27-31 (9am to noon)
    • online registration open now - be sure to select 2017-2018 school year
  • Kindergarten Orientation Parent Meeting - 9am on March 30th
  • Grade 1-6 Registration- April 3-6 (9am to noon)
    • online registration open now - be sure to select 2017-2018 school year
  • K-6 School Choice Application Window - April 3-28, 2017

Registration Requirements

  • County Birth Certificate or Passport
  • Child must be 5 years old on or before 09/01/17 for Kindergarten 
  • Address verification in the Melinda Heights attendance area
  • Must have: Closing escrow papers, grant deed, or lease agreement AND gas/electric utility bill
  • Immunization record with documentation from physician's office.  
  • Parent or Guardian picture ID
  • Printed Copy of Online Registration Forms

Current Vaccinations:

  • MMR: Two doses - both on or after first birthday
  • Hepatitis B Immunizations: 3 doses at any age
  • Varicella (Chicken Pox): One dose or physician documentation of history of disease (for first time entry to California school through age 12 yrs).
  • Polio: Four doses at any age
  • DTP:  Five doses at any age
  • Vaccination Law Update

First Grade: (due within 90 days of registration)

  • Physician Assessment Form:  Must be completed by a physician, have physician's signature & physician office stamp on exam.
  • Oral Health Assessment Form:  Must be completed by dentist, have dentist's signature & dentist office stamp on exam form. Requirement in kindergarten or first grade, whichever is his/her first year of public school.


Online Registration

*Parents of NEW Melinda Heights Elementary students will register online for 2017-18.

If your student is currently (or previously) enrolled in any SVUSD school or program (including Transitional Kindergarten at an SVUSD school), there is no need to register online.  Paperwork will be given to you for registration in the school office.

Step 1:

Collect all your student immunization records and emergency contact information; and on your computer, go to:

*Your child must be 5 years old on or before September 1, 2017 to be eligible for registration.

Step 2:

Carefully follow the directions and complete your students’ registration.  (This will take approximately 30 minutes). Review your information, make any corrections and then submit.  (IMPORTANT: Once you’ve submitted you cannot go back to make corrections).  Make sure you print a copy and bring it into our office as soon as possible to ensure that we activate your student and their records.

Step 3:

When you come to our office you will need to bring:

  • Printed copy of your online registration forms
  • Proofs of residence - Closing Escrow papers, Grant Deed, or Lease; AND one of the following: current - gas, electric or water utility bill.
  • Parent or guardian picture I.D.
  • Student's birth certificate or passport
  • Student's 5-year immunization record.  

The Office will have the following additional forms which will require your attention.
