The following information is required for 

School Registration 2009-2010

-County Birth Certificate or Passport

-Child must be 5 years old on or before 12/1/09 for Kindergarten 

-Address Verification in the Melinda Heights attendance area

Must have: Closing escrow papers, grant deed, or lease agreement AND gas/electric utility bill

-Immunization Record with documentation from physician's office.  

Must have doctor's signature and doctor's office stamp on the immunization record.

Current Vaccinations:

-MMR:Two doses - first dose must be after 1st birthday
-Hepatitis B Immunizations: series of 3 doses required for Kindergarten & 7th grade
-Varicella (Chicken Pox):One dose or physician documentation of history of disease (for first time entry to California school through age 12 yrs).

-Health Exam for School Entry - Completed by Physician (including physician's signature and office stamp) 

Kindergarten: 

    Completed within the 6 months prior to entering  Kindergarten 

First Grade: 

     Completed within the 18 months prior to entering First Grade

     PPD (TB) Mantoux skin type test required -Must have date given, date read, mm duration, doctor's signature and doctor's office stamp on immunization record. This must be done within 18 months of           entering First Grade.

School Start Date for 2009: September 10th
